Dr. Blair Peters aka the Queer Surgeon who performs gender surgeries and is an ASPS member has provided some insight into this. You can see in the link below that he said the broader membership of the organization did not know about this statement nor approve of it and that actual surgeons who perform these surgeons were not consulted. Additionally, he believes this to be a 100% political decision that does not represent the view of most ASPS members etc.
The AMA also caved. This isn't going to stop with gender affirming surgery for minors, or with minors in general. They're going to continue to push these organizations until they come out against all GAC, full stop.
the thinking is that by saying "under 19" they can ban care for adults & establish a precedent that will allow them to ratchet the age up until it's completely banned
Also, the federal attempts at a ban apply to HRT as well. So it isn't even internally consistent, because they want to ban HRT until 19 (currently) as well. It's not meant to make sense, it's meant to harm trans people.

Aren’t you considered an adult and capable of making your own decisions at 18? So no buying cigarettes or enlisting in the military until 19?
i think the thinking is that these people dont want people on hrt until 18, and traditionally doctors wait until a year on hrt to do surgery

well obviously that is the end goal but for now its very internally consistent and with the standards to say hrt at 18 surgery at 19
